Preparing for your first visit
Prior to your scheduled visit, we kindly ask you to download and complete the medical questionnaire. The information you provide will help Dr. Lee properly assess your condition(s) and develop an action plan for management and treatment as it fits with your other medical problems, medications, and general approach to health. The questionnaire will also serve to lay the foundation of a detailed health history, which she will review with you during your first visit. We also encourage you to bring copies of any pertinent medical records such as lab work, pathology reports or clinical notes for Dr. Lee’s review during your first visit. We assure you that the information in this questionnaire will be kept strictly confidential and will not be released without your written permission.
On the day of your visit
During your initial consultation Dr. Lee will perform a comprehensive review as indicated by the complexity of your symptoms. She will review your completed medical history form, perform a complete head to toe skin exam, including nails and hair. You may require additional laboratory testing or further study of your lesions under the microscope.

A valid government issued photo ID
Your insurance card
Any pertinent medical records such as lab, pathology reports or clinical notes, etc.
By seeking Dr. Lee’s consultation, please be aware that she is an OUT-OF-NETWORK provider for all insurance other than "Traditional" or "Original" Medicare. Dr. Lee is not a provider for any Medicare Advantage plan. She is OUT-OF-NETWORK for any Medical plan that is affiliated with a Medical group or HMO. She may not see patients with Medicare Advantage or if your Medicare plan is assigned to a medical group or HMO.
For our patients’ convenience, our practice bills all major insurance plans such as: "Traditional" or "Original" Medicare, Blue Cross, Blue Shield, Cigna, Universal Healthcare, BlueCare, Tricare, Signature Network, and HealthNet. However, please note that Dr. Lee is NOT contracted with these non-Medicare insurance companies.
